Wisdom tooth pain · Greater Kailash 1, Delhi
A painful wisdom tooth is usually one that has too little room to come through, and the pain often comes with a swollen, sore flap of gum. At Orion Dental Clinic, a dentist in Greater Kailash 1 (GK 1), South Delhi, this page explains what causes wisdom tooth pain, what you can do at home, which signs mean you need help quickly, and what the dentist can do, in plain language.
Care is planned by Dr Anupam Sinha (BDS, MDS) and Dr Natalia DeSilva (BDS, MDS). If you have swelling that spreads, or trouble swallowing or breathing, call 112 or go to a hospital now. Ask as many questions as you need.
These statements come from NHS patient information and systematic reviews, listed at the end of the page. Your dentist will explain what applies to your own tooth.

Quick answer
Wisdom tooth pain at Orion Dental Clinic in Greater Kailash 1, Delhi is examined with an X-ray to find whether infection, decay, gum swelling or lack of room is the cause. Dr Anupam Sinha and Dr Natalia DeSilva explain the options. Pain with a bad taste or fever needs a dentist urgently.
01 Overview
Wisdom teeth may cause problems if they lack space to come through fully, for example when blocked by another tooth or partly covered by gum. The NHS lists pain, swelling, gum infection, trapped food, decay, gum disease, cysts and abscesses.1 An infection can start around a wisdom tooth that has broken the skin but lacks room to come through, and that is one of the common causes of toothache.2
Infection is the pain to take seriously. A dental abscess is a build-up of pus that causes intense pain, redness, a bad taste, sensitivity to hot and cold, trouble opening the mouth and swelling of the face or jaw, and it needs urgent dental treatment.3 The NHS also says to seek an urgent dentist appointment if wisdom tooth pain comes with a bad taste, a high temperature or feeling unwell.1
A wisdom tooth that is not causing problems is usually left alone and checked at routine visits.1 Our page on wisdom tooth removal explains surgery in detail, and our page on tooth pain explains other causes.
Wisdom tooth pain often means a tooth that has no room, and pain with fever or a bad taste means infection.
02 Candidacy
The NHS gives these guides, and the dentist decides what is needed after an examination.12
Pain with a bad taste, fever or swelling that spreads should not wait.
03 Options
Treatment depends on the cause, and the first aim is to ease the pain and clear any infection.
The NHS advises painkillers such as ibuprofen or paracetamol, warm salt water rinses for adults, soft foods, a soft toothbrush, and avoiding sweet, very hot or very cold food and smoking.2
While you wait 2
A dentist drains an abscess, usually under local anaesthetic, and painkillers or antibiotics may be given.3 In healthy people having impacted wisdom teeth removed, antibiotics may reduce infection, but the evidence is low-certainty and resistance is a concern, so the dentist decides.4
Urgent 3
If the tooth is not causing problems, the NHS says it is usually left alone and monitored.1
| Home care | Treating infection | Removal | |
|---|---|---|---|
| What it involves | Painkillers, salt water, soft food 2 | Draining pus, antibiotics only if needed 34 | Surgery, usually under local anaesthetic 1 |
| When | While you wait for an appointment 2 | Pus, swelling, fever or bad taste 13 | Repeated trouble 1 |
| Main limit | Does not treat the cause 2 | Antibiotic evidence is low-certainty 4 | Risks include dry socket and numbness 1 |
Home care eases the pain, and the dentist treats the cause.
04 Procedure
Every case is individual, so your steps may differ. This is the usual order.
The infection is dealt with first, and the decision about the tooth follows.
The dentist asks where it hurts, how long, what makes it worse, and whether you have swelling, a bad taste, a fever or trouble opening your mouth.13
The dentist checks whether the tooth has come through, whether the gum is swollen or infected, and whether food is trapped or the tooth next to it is decayed.1
An X-ray shows how the tooth lies, whether it is blocked by another tooth, and where its roots are in relation to the jaw.
If there is an abscess, it is drained, usually under local anaesthetic, and antibiotics are given only if the dentist judges them necessary.34
You talk through leaving the tooth, which is usual when it causes no problems, and removing it if it keeps causing trouble.1
The area is numbed, the gum is cut if it covers the tooth, the socket is widened and the tooth is taken out whole or in pieces, usually in no more than 40 minutes.1
A check shows whether the pain has settled and, after removal, whether the socket is healing.
05 Aftercare
These are the NHS self-care steps. They ease pain, but they do not treat the cause, so you still need a dentist.23
Ibuprofen or paracetamol can help, within the recommended dose. Children under 16 should not take aspirin.23
Rinse with warm salt water, half a teaspoon of salt in a glass of water, if you are an adult. Use a soft toothbrush and avoid flossing around the sore tooth.2
Call the clinic on 098115 01995 if the pain lasts more than two days or does not ease with painkillers, or if it comes with a swollen cheek or jaw, a bad taste, a high temperature or feeling unwell. Go to a hospital emergency department if you have swelling that spreads across your face or neck, a swollen or painful eye, or trouble breathing, speaking or swallowing, and ask someone to take you rather than driving yourself.
Home care eases the pain for a while, and the dentist treats the cause.
06 Limits and risks
Pain at the back of the mouth can come from other places. These are situations where something else may be going on:
In these cases the dentist looks at the whole mouth, may treat another tooth, or may refer you to a doctor.
An abscess will not clear on its own. It needs urgent dental treatment, and swelling that affects breathing, speaking or swallowing, or the eye, needs a hospital emergency department.3
Antibiotics help only a little. In healthy people having impacted wisdom teeth out, about 19 people needed antibiotics to prevent one infection, the evidence is low-certainty, and the Cochrane authors warn about antibiotic resistance.4
Removal has risks too. The NHS lists dry socket, infection and nerve damage causing numbness or tingling in the tongue, lips or chin, which usually improves but can last weeks or months.1
Smoking raises dry socket risk. Combined dry socket incidence was about 13.2% in smokers and 3.8% in non-smokers across 11 studies.5
A good plan finds the real cause of the pain and is honest about the risks of each choice.
07 Your visit
At your first visit, a dentist at Orion Dental Clinic asks where the pain is, how long you have had it, what makes it worse and whether you have swelling, a bad taste or a fever. You are asked about your health, medicines and allergies.
The dentist examines the tooth and the gum, and takes an X-ray to see how the wisdom tooth lies. You then talk through what is causing the pain, how any infection will be treated, whether the tooth should come out or be watched, and what the risks are, and nothing is done until you agree.
The first visit is for easing the pain and finding the cause, and you decide about the tooth.
08 Why this clinic
You meet the doctors who examine your tooth, and you can ask them whether it needs to come out.
