All-on-4 Dental Implants in Greater Kailash 1, Delhi

Orion Dental Clinic is a dentist in Greater Kailash 1 (GK 1), South Delhi. All-on-4 is a way of replacing the teeth in one jaw with a fixed set of teeth that rests on four implants, instead of one implant for every tooth. This page explains how it works, who it may suit, how long it takes, how recovery goes and what the research does and does not show, in plain language, so that you can decide with confidence.

Treatment is planned by Dr Anupam Sinha (BDS, MDS) and Dr Natalia DeSilva (BDS, MDS). For the wider picture, read our guide to full mouth dental implants. Ask as many questions as you need before you decide.

Quick answer

All-on-4 at Orion Dental Clinic in Greater Kailash 1, Delhi replaces the teeth in one jaw with a fixed set held by four implants. Dr Anupam Sinha and Dr Natalia DeSilva plan each case after an examination and a 3D scan. It suits some people, not all, and the final teeth are fitted after months of healing.

01 Overview

What is All-on-4, and how does it work in Greater Kailash 1, Delhi?

All-on-4 is a treatment concept for a jaw that has lost most or all of its teeth. Four titanium implants are placed in the jawbone, and a full arch of fixed teeth is attached to them. The two implants at the back are often tilted, which lets them sit in the bone that is available without a bone graft in some cases. After the implants are placed, a fixed temporary set of teeth can sometimes be attached soon afterwards while the bone heals, and the final teeth follow later.1

In the research the concept is called all-on-four, and it is one design within the wider field of full mouth dental implants. A review of 24 studies describes it as a predictable way to treat a jaw with little bone in patients who prefer to avoid regenerative procedures such as bone grafts.1 The same review says the evidence is limited, because follow-up was short and many patients dropped out of the studies.

There is also a six-implant version, which spreads the load across more posts: see All-on-6. Which one suits you depends on your bone, your bite and your general health, and your dentist will explain the choice after a scan.

All-on-4 is one design among several, and the research on it is promising but limited.

02 Candidacy

Who can have All-on-4 in Greater Kailash 1, Delhi?

All-on-4 suits some adults who have lost most or all of the teeth in a jaw, or whose remaining teeth cannot be saved. It is a major treatment, so your dentist decides after an examination and a 3D scan, and may suggest treating other problems first.3

Who All-on-4 may suit

  • Adults whose jaws have finished growing and who have lost most or all teeth in one jaw, or whose remaining teeth cannot be saved.
  • People whose bone is limited and who would prefer to avoid a bone graft, if the scan shows enough bone at the front of the jaw.1
  • People with healthy gums who are ready to clean beneath and around the fixed teeth every day.3
  • People who are fit enough for oral surgery, whose health conditions are under control and who can attend many visits over several months.

Who needs extra care or a different plan

  • Smokers. Smokers have a much greater failure rate, and NHS guidance asks patients to stop at least six months before treatment.3
  • People with gum disease that has not been treated. Gum treatment comes first.3
  • People who grind their teeth, because grinding can overload implants.3
  • People with health conditions or medicines that affect healing, or who have had radiotherapy to the jaw.4

Being a good candidate depends on how much bone you have, where it is, and your gums and general health.

03 Options

Which options replace a full arch in Greater Kailash 1?

When most or all teeth in a jaw are missing there are several designs, and the number of implants is one of the main differences.

All-on-4 fixed full arch

Four implants, often with the back two tilted, carry a fixed set of teeth that only the dentist can remove.

Four implants per jaw 1

All-on-6 fixed full arch

Six implants carry the fixed set, which spreads the biting load over more posts and needs room for more of them.

Six implants per jaw

Implant-retained overdenture

A removable denture clips onto a few implants, so it is steadier than an ordinary denture. You take it out to clean it, and the clips can wear out.3 See dentures.

Removable, with parts that wear

A fixed temporary set of teeth is sometimes fitted soon after surgery, which people call same-day teeth. Whether this suits your bone is decided after a 3D scan, and the final teeth are still made after healing. Read more about immediate implants.

Four or six implants is a decision for your dentist, based on your bone and your bite, and not a fixed rule.

04 Procedure

How is All-on-4 placed, step by step?

Every case is planned individually, so your steps may differ. This is the usual order.13

A fixed set may be attached early, but the bone still needs months to heal before the final teeth.

  1. Examination and 3D imaging for All-on-4

The dentist asks about your health and medicines, examines your mouth, gums, any remaining teeth and your current denture, and takes a 3D scan. This shows how much bone you have, where it lies and where nerves and sinuses are.

  2. Treating or removing teeth that cannot be saved

Gum disease and decay are treated first, and teeth that cannot be saved are taken out, which is tooth extraction. Removal may happen before the implants or at the same visit, depending on your plan.

  3. Implant surgery for a full arch

The area is numbed with a local anaesthetic, and some people also have sedation. The dentist places four implants, tilting the back two where the plan calls for it, and closes the gum around them. Ask about sedation dentistry if you are anxious.

  4. Temporary teeth while the bone heals

In suitable cases a fixed temporary set of teeth is attached soon after surgery. Otherwise you wear a temporary denture, which may need adjusting. You are given written care instructions and a soft food plan.

  5. Healing and osseointegration

The bone bonds to the implants over about three to six months, and longer if a graft was needed.3 Review visits check the gums and the temporary teeth.

  6. Fitting the final fixed teeth

When the implants are firm, impressions are taken and the final set of fixed teeth is made to match your bite and your face. It is fitted, checked and adjusted at later visits.

  7. Reviews and cleaning beneath the fixed teeth

NHS guidance advises monitoring every three to six months over the long term.3 Your dentist checks the gum around each implant, the bite and the teeth, and shows you how to clean beneath them.

05 Aftercare

Recovery after All-on-4 surgery in Greater Kailash 1

Recovery has four phases. Your dentist will give you written instructions that fit your treatment, and those come first.

The first 48 hours

Bleeding, swelling and discomfort are common.3 Keep the area clean, eat soft, cool food, rest, and take your medicines as prescribed. Do not smoke.

The first week

Swelling and bruising settle. Stay on the soft food plan, rinse gently if you are told to, and attend your follow-up visit for stitches and a check of the temporary teeth.

Weeks 2 to 12

The implants are bonding to the bone. Brush as normal, clean the temporary teeth as shown, keep every review visit and avoid tobacco.

Long term

Brush twice a day and clean beneath the fixed teeth with the interdental brushes or aids your dentist shows you. Attend check-ups, which NHS guidance puts at every three to six months.3

When to call us

Ring the clinic on 098115 01995 the same day if the temporary teeth or a screw feel loose or break, if bleeding will not stop after 20 minutes of firm pressure, or if swelling is still growing after day three. A fever, pus, a bad taste or pain that your medicines do not control also needs a call. If you have trouble breathing or swallowing, or swelling that spreads towards your eye or neck, go to a hospital emergency department.

Follow your written instructions, keep the area clean and call early if something feels wrong.

06 Limits and risks

When is All-on-4 not the right choice?

All-on-4 involves major surgery, and it does not suit everyone. A good dentist is willing to say no, or to wait. These are situations where your dentist may advise a different option, a delay, or advice from your other doctors:

  • Gum disease that has not been treated.
  • Too little bone at the front of the jaw, or bone that cannot hold four implants firmly.
  • Smoking, if you are not ready to stop.
  • Health conditions that are not under control, or medicines that affect bone healing.
  • Radiotherapy to the jaw in the past.
  • Teeth grinding that is not controlled.
  • Not being able to attend the many visits and check-ups the treatment needs.

In these cases six implants, a denture that clips onto implants, or an ordinary denture may be the better first step, or implant treatment may begin after another problem has been treated.

Risks and complications

The evidence is limited. A review of 24 studies, covering follow-up of at least three years, reported 99.8 percent implant survival beyond 24 months. Its authors also say that the studies were small, follow-up was short and many patients were lost along the way, so the figure should be read with caution.1

The number of implants may matter. A larger review of 54 studies of fixed and removable sets estimated that 97.9 percent of upper-jaw implants and 98.9 percent of lower-jaw implants were still in place after five years. In the lower jaw, fixed sets on four implants lost more implants than fixed sets on five or more.2 That is one reason your dentist may suggest six.

The gum around an implant can become inflamed. A review of 11 studies found that on average 43 percent of patients had inflamed gum around an implant and 22 percent had the more serious form that destroys bone, with wide differences between studies.5 Inadequate cleaning can lead to long-term bone loss.3

Surgery has its own risks, and parts wear. Swelling, bruising and discomfort are common for a few days, and infection can occur. Altered feeling or numbness in the lip or chin is very rare and usually settles.3 Teeth, clips and screws can loosen or wear out and may need repair or replacement.34

A good plan names these risks before treatment starts, and a good clinic keeps checking the implants and the teeth for years afterwards.

07 Your visit

What happens at your first All-on-4 visit?

At your first visit, a dentist at Orion Dental Clinic asks about your current teeth or denture, what you find difficult, and your health, medicines and smoking. Bring any recent X-rays, your denture if you wear one, and a list of your medicines.

The dentist examines your mouth, gums and remaining teeth and takes a 3D scan of both jaws. You then hear whether the bone allows four implants in each jaw, whether a design with six would suit you better, whether temporary teeth are possible after surgery and what the risks are.

The first visit is for questions and a plan, and you can ask for a second opinion.

08 Why this clinic

Why choose Orion Dental Clinic for All-on-4 in Greater Kailash 1?

You meet the doctors who plan your treatment, and you can ask them why they would choose four implants or six for your jaw.

Two doctors who see implant patients

Dr Anupam Sinha and Dr Natalia DeSilva both treat implant patients, and past patients name both of them in their reviews.

A 3D scan before the plan

Planning depends on a 3D scan. Imaging is available at the clinic, and Dr Natalia DeSilva has studied 3D imaging with CBCT.

Visiting from elsewhere

People come to the clinic from other cities and countries, and Dr Natalia DeSilva also treats Russian-speaking patients.

Ask why four implants, or six, is the right plan for your jaw.

11 Travelling patients

Travelling to Delhi for All-on-4?

All-on-4 can bring a fixed temporary set of teeth early, but the final teeth follow only after the bone has healed over several months. For patients who live elsewhere, that usually means an evaluation, a surgery visit, healing at home and a return trip for the final teeth.13

Tell us where you live and how long you can stay in Delhi, and we will explain what a short visit can cover and what has to wait. You can read how dental tourism in Delhi works, or go to our page for international patients having dental implants.

Plan All-on-4 as a staged treatment over months, even when temporary teeth are fitted early.

12 Questions

All-on-4 questions patients ask in Greater Kailash 1

What does All-on-4 mean?

It means a full arch of fixed teeth carried by four implants in one jaw, with the back two usually tilted so that they can use the bone that is available.1 It is also written all-on-four.

Is All-on-4 better than All-on-6?

Neither suits everyone. In a review of 54 studies, lower-jaw fixed sets on four implants lost more implants than those on five or more,2 but your dentist plans from your bone and your bite, and some jaws cannot take six implants.

How long do All-on-4 implants last?

A review of 24 studies found 99.8 percent of implants still in place beyond 24 months, but its authors say the evidence is limited and the follow-up short.1 Over five years, a wider review estimated 97.9 percent survival in the upper jaw and 98.9 percent in the lower jaw for all fixed and removable designs.2

Can I have teeth on the same day?

In suitable cases a fixed temporary set is attached soon after surgery. The bone still needs about three to six months to bond before the final teeth are made.3

Is All-on-4 surgery painful?

The area is numbed with a local anaesthetic, and the aim is that you feel no pain during the procedure. Afterwards, swelling, bruising and discomfort are normal for a few days.3 If you are anxious, ask about sedation or read about dental anxiety.

How long does the whole treatment take?

NHS guidance says the whole process could take 6 to 12 months,4 and another NHS hospital says the total time is usually at least one year.3 Your dentist will give you a timeline for your own mouth.

Do I need a bone graft for All-on-4?

The concept is described as a way to treat a jaw with little bone without regenerative procedures, in patients who prefer to avoid them.1 Whether you need a graft depends on your scan, and a graft adds at least four months.3

Can I have All-on-4 if I live outside Delhi?

Often yes, but the treatment is done in stages over many months. Your dentist will say which steps need you in the clinic and which can wait. Message us to plan the visits.
